Blue Springs Waiver-Tournament

I acknowledge that participation in activities, including but not limited to games, leagues, tournaments, practices, and use of facilities at Centerline Volleyball, involves inherent risks. These risks may include, but are not limited to, serious bodily injury, permanent disability, paralysis, or death, as well as property damage. If signing for a minor, the parent/guardian assumes all risks.

I voluntarily assume all risks associated with my participation. I agree to abide by all rules and guidelines set by Centerline Volleyball. This waiver releases any claims whatsoever involving any Centerline enterprise (CBVLLC, BadgerWoods Group LLC, CNVB Enterprises, and Centerline North LLC ect..) I understand that failure to follow rules or unsafe behavior may result in removal from the facility without refund. By providing my email address above I consent to emails sent by Centerline Volleyball.
